  • A good faith statement that the removal was mistaken
  • Your name, address, and consent to jurisdiction in New York State
  • Your signature

Upon receipt, we may reinstate the material within 10–14 business days unless legal action is initiated by the complainant.

We reserve the right to block access or terminate user accounts (if applicable) for repeat offenders or those found submitting fraudulent DMCA claims.
